Take a look at this chart and see if it helps you with a diagnosis.   I am concerned about it spreading like this. It is likely to be an infection, needing antibiotics for treatment. See your health care practitioner tomorrow. http://familydoctor.org/545.xml

They don't look like blisters, just tiny hives. He says they're not itchy or painful. I noticed that some of them are just red and scaly, no scab.

Psoriasis is a possibility. Hydrocortisone cream would be the thing to try, as that can moisturize and decrease the inflammation. If at any point the bumps are purple (not just the scab) or blood red, he needs to see a physician immediately. If the rash hasn't improved in 3 days of using the hydrocortisone 3 times a day, he also needs to see a physician.

do you think a dermatologist could help?

A dermatologist would be great, but it can take a long time to see a specialist sometimes. So if that is the case where you are, use the hydrocortisone, if it hasn't cleared in a week, he needs to see a doctor. You might go ahead and schedule an appointment with a dermatologist. If the rash clears, you can cancel it.
