When I pivot on my right leg I feel a sharp pain in

When I pivot on my right leg I feel a sharp pain in the top of my middle of my hip like it is coming out of the socket and when I stand up straight my right foot is rotated outward at a 45 degree angle. When I try to place it paralell with the left foot it hurts in the same place. is this serious? what is the problem?
