Many times a surgical approach is associated with many different procedures - e.g. a laparotomy can be done to accomplish a cholecystectomy, an intestinal repair, an appendectomy, hysterectomy, etc. etc. etc. It is well-known by the insurer that the approach is needed for the procedure. So rather than code for two separate procedures, the two are bundled into one. The answer to the second part of the question may be similar, but I am not sure.
