Wondering what are my chances of being pregnant?

I've been taking Tri-Sprintec for a year very consistently, at exactly noon every day. However, last week I went out of town for two days and forgot my birth control! So I missed the third and fourth pills in the first week of the pack. As soon as I got home, I took the two pills I missed and the next day I took two more pills so now I'm all caught up. But since last week and now, I've had sex twice. Both times my fiance stopped mid-intercourse to put on a condom. I'm concerned about the fact that we engaged in unprotected intercourse within a few days after I missed my pills. Also, the most recent time, the condom slipped off as he pulled out after ejaculating and I think some of his semen may have been in contact with the opening of my vagina. This all seems risky to me and I'm terrified...could I possibly be pregnant? And if so, is it too late to take Plan B??
