Question

Pre-mature infant has surgery for a duodenal artresia.

Pre-mature infant has surgery for a duodenal artresia. Stays in NICU for four weeks. Mother feeds baby Pregestimil for 3.5 weeks and then starts mixing half pregestimil and half regular Enfamil. Baby is back in Intensive care with large amounts of bile excreted from mouth, npo and they are calling it flu induced ileus. What do you think?

Already Tried:
Surgery on duodenal web. Upper GI showed slow movement with no obstruction.

Greetings.

Since there is no obstruction (anatomical obstruction is ruled out by GI series), with slow movement of the gut, it is classified as paralytic ileus (paralysis of intestines). This is physiological obstruction and usually self limiting and should remit in 3-4 days. Does not look like complication of surgery.

What is paralytic ileus and why does it occur. Should tests be ran to check the actual bile duct and its functioning?

How do we know when it is the right time to try and feed again so we do not have another incident where he chokes to the point of having aspirations?

Hello,

Absence of abdominal distension, return of bowel sounds, normal passage of flatus and absence of vomiting are the signs, which indicates that things have returned back to normal.
