It is possible that diverticular disease can come back. Often surgery is done after a course of antibiotics has been unsuccessful. Your doctor may be able to attempt a course of antibiotics first before jumping to surgery.
The disease is based on outpouching of areas of the bowel and can be caused by a number of reasons including high pressure on the colon (perhaps you are overweight or have high pressure on your bowel like chronic constipation), also low fiber diet (so it is good that you are now taking fiber). These diverticula or outpouches can occur anywhere, and while they usually happen in the left side of your colon, they can occur elsewhere and cause either bleeding or pain and infection, as it sounds like you are experiencing now.
