I had hernia surgery 3 years ago and have had a larger right testicle ever since. It usually doesn't hurt but sometimes i can feel pain in my lower back which connects to this larger testicle. Is this normal and should i get it checked out?

This doesn't appear to be normal. I would have it checked out. The pain in the lower back can contribute to testicle problems and vice versa.. But without a proper evaluation it would be impossible to say what is going on. So do make an appt with your doctor just to be on the safe side.
