Studies have shown that men with early stage prostate ca have used low-dose rate brachytherapy with good results. Most have shown the ability to still be able to achieve an erection and have a very low rate of incontinence 1 year post treatment so I would highly recommend that he go with the radiation therapy before I would recommend the surgery, especially if he is extremely active andf has no other health problems besides the hyperlipidemia. If they are allowing him to make a choice without a recommendation then that most likely means that they feel both would be a good choice for him but studies have shown that there is less of a chance of incontinence after the radiation therapy and if he is extremely active I feel this would be the best route for him to go. I know there is an article that I read on this subject, I will try to find this and post it for you as well. With the surgery there will be longer healing time and there is a higher chance of incontinence so my recommendation would be the radiation therapy by far since he this is in the early stages and the radiation is able to be used by low-dose.
