Okay, now I know where you are coming from. You want to check syphilis off the list of potential causes.
You are correct, tertiary stage is tough to pinpoint because the symptoms all depend on which organ system has been affected.
When you take the RPR, if it comes back negative, you should be in good shape. Getting negatives on both tests at different times will give you a very good reading.
If you are still not satisfied with negative results on both of those test, you can request a FTA-ABS test. This is a blood test, it stands for (Fluorescent treponemal antibody absorption test). Anyway, it detects bateria directly related to syphilis. This test is usually only done when either the VDRL or RPR come back positive, BUT it is also used when checking for tertiary and there are fears of a false negative.
And Finally, you can do a spinal fluid exam. CSF (cerebrospinal fluid) is obtained through a spinal tap. It's usually for testing bacterial meningitis, but can also be used to check for tertiary syphilis. Really though, if you do the RPR, and the FTA-ABS, this shouldn't be necessary.
