where do I go from here? after 4 yrs of gastro problems, including upper stomach pain, nausea, diahrrea, heartburn, severe chest pains, among others, I have been dianosed as having reflux, gald stones, indigestion, esopheal spasms, after nexium for 2 years, gald bladder removal, 2 endoscopies, and an mrcp, I still have these problems and they are becoming more frequent. I've been on protonix for 5 months, am iron deficient, witha ferratin level of 7-10 after receiving IV iron treatments. is this just something i have to live with, without any effective medications? I know this is a lot to take in, but I'm lost...please help if you can

protonix, phenegran, diet...I also take lamictal, paxil, & diazapam for bi-polar adepression/anxiety disorder. gall bladder scan & removal, endoscopy, mrcp (results small cyst on liver), I think that's all.

The iron deficiency should be investigated , aside from the blood tests and endoscopy. Low iron could contribute to anaemia which has been associated with chest pain and shortness of breath.

Other causes of low iron are : undiagnosed bleeding, heavy periods, malabsorption syndromes, and increased retension of RBC in spleen leading to enlargement of spleen. This would cause left sided abdominal pain but would be identified by a CT scan or physical palpation.

The chest pain should also be properly investigated to rule out cardiovascular problems.

With your history, we may not be able to cure but we would look to manage your symptoms. For example using an alginate liquid for heartburn will coat the oseophagus and stomach contents reducing heartburn. Some iron supplements can cause nausea, constipation or diarrhoea so you may have to consider how your iron deficiency is managed. IV injections are not very convenient but they may reduce GI symptoms.

Using hyoscine or mebeverine can reduce muscle spasms in the intestines and often help with abdominal pain.

Referral to a dietitian who can spend time working out a proper blanced diet, which reduces some of your problems may also be worth doing.

I'm sorry I do not have a definative diagnosis but with time, you should be able to get your symptoms under control.

I have been receiving IV iron supplements. Nothing has revealed any internal bleeding, my mrcp investigated liver, pancreas, and bile ducts. The gall bladder scan revealed only gall stones (gall bladder has been removed) nothing was shown to be wrong with spleen, duodenum or pancreas. I feel that my initial question relayed all of these results. Your response,while detailed, was already investigated and ruled out by my physicians. Thank you for your time, but I cannot accept this answer.
