What would cause the bowel movements to flatten instead of being completely round?

Internal and/or external hemorrhoids and partially obstruct (e.g. narrow) the rectum/anus which can result in changes in the shape of expelled stool.

Simple, at home measures -- such as increasing fluid and fiber intake in order to soften your school and lessen the need to strain can bring about reduction of hemorrhoids.

Sorry for the delay - I just noticed the question:

A change in the caliber (small diameter) or shape of the stool MAY be an indication of some narrowing of the rectum or sigmoid colon. There are a number of things that can cause narrowing of the rectum/sigmoid - the most serious of these is a tumor.

Other things that can cause this - swelling from an infection or inflammation of the colon (colitis)

This may indeed be a serious condition - and you should not ignore this problem. I would suggest that you see a gastroenterologist to have a sigmoidoscopy/colonoscopy to evaluate the cause of this problem.
