I need a specialist to solve problem with my husband''s swallowing problems, he is constantly choking, has been told he has no gag reflex, he has an overproduction of saliva that he is also choking on, just to mention a few symptoms. we have been through GI specialist, neurologists, ENT throat specialists,and more but nobody can come up with any answers. I,m afraid he is going to have a heart attack from all this coughing that keeps him from sleeping. he does have chest pain quite frequently as well as he aspirates frequently at night from these secretions. Please help, we don''t know where to turn next. Joan C.   Bradenton,FL

work up for neuromuscular conditions, swallowing studies, MRI brain,CT thorax,throat.upper endoscopy,multiple visits with physicians in Sarasota,FL ,GI,ENT, neurologists, rheumatiod/arthritis spec. for Sojurn syndrome w/u which showed nothing.

it has been going on for over 18 months, the only finding on upper endoscopy was a small possible constriction but not notable enough for dilation or anything, our GI Doc basically gave up,after doing most of the routine tests , mononmetry studies, x-ray swallowing studies, nothing abnormal,, He has been on Protonix for the last 18mos and it really hasnt made a big difference. one thing that I can't get an answer to is if the two herniated thoracic discsT3-4 and T7-8 could have anything to do with these symptoms?   He reallly only tolerates soft diet and regardless of what and when he eats the coughing and choking is worse after eating, when food gets stuck, or comes back up later. He really feels like he is going to die from all this coughing. He recently injured his back and the muscle relaxants he was taking for that did seem to also quiet the coughing for a short while, but he is the type who doesnt want to just have more dru;gs thrown at him, he wants to find a cause or answer.

All the symptoms individually sound like a Case of GERD, many of them sound like Sjogrens syndrome ( but blood tests and biopsies were negative. there are definitely esophageal spasms going on , but also loss of taste, extra saliva production and the very frequent aspirations, which lead to fevers and congestion chronicly, please advise to what kind of specialist to seek out next, we are really at the end of our rope. Non-stop coughing, lack of sleep and depression from being so totally frustrated with the situation is consuming our lives.

Hello,

Thank You for a clear elaboration of your husband's symptoms and tests.

I would like to suggest that you go for an Endoscopic Ultrsound of the Esophagus.

This will give us a clearer idea about the constriction ring.

This is the only procedure that can demonstrate any irregularity that is within the wall of the esophagus like a subtle leiomyoma ( a muscle tumor of the oesophagus ).

I would also like to suggest a Mediastinoscopy, this involves visualization of the structures behind the breast bone, but outside the Oesophagus and Trachea.

Any mass arising from this area can compress on the Oesophagus and produce similar symptoms as you describe, and still not show up on the studies that you have done already.

I think a PET - FDG scan would be warranted in this case, as the diagnosis is not clear,
we need to consider possibilities of a primary lesion at a different site from the Esophagus, producing a secondary growth, that produces these symptoms.

The Herniated thoracic discs would not cause generally cause a difficulty in coughing,
with the exception of these being large anterior herniations of the discs causing a compression of the esophagus.
