ear infeaction for one month, both ears

ear infeaction for one month, both ears right ear has a ruptured eardrum and the left ear has swelling, tenderness, clear discharge , loss of hearing and there seems to be a knot behide the left ear 1 1/2 down left side of the neck.

earwax removal and several visits to the doctors they dont know whats wrong
